Who Are We?

The Maryland Collaborative Care Transformation Organization, Inc. (MDCCTO) is a Care Transformation Organization participating in the Maryland Primary Care Program (MDPCP), an initiative of the Maryland Total Cost of Care All-Payer Model through the Centers for Medicare & Medicaid Services (CMS) and the Maryland Department of Health (MDH).

As a MDPCP Participant Practice, you are part of a program that helps to deliver advanced primary care throughout the state. A Care Transformation Organization assists practices in their patients’ care and helps them see that patients get the right care, in the right place, at the right time.

Beneficiaries can connect to a wide range of social services through the Community Connections Help Line. With one phone call, speak to a Peer Coach who will listen to your needs and refer you to existing resources all over the country or right in your local area. These social services include transportation, medical assistance, and healthy living programs.
