Who Are We?

The Maryland Collaborative Care Transformation Organization, Inc. (MDCCTO) is a Care Transformation Organization participating in the Maryland Primary Care Program (MDPCP), an initiative of the Maryland Total Cost of Care All-Payer Model through the Centers for Medicare & Medicaid Services (CMS) and the Maryland Department of Health (MDH).

As a MDPCP Participant Practice, you are part of a program that helps to deliver advanced primary care throughout the state. A Care Transformation Organization assists practices in their patients’ care and helps them see that patients get the right care, in the right place, at the right time.
