Maryland Collaborative Care Transformation Organization, Inc.

Who Are We?

The Maryland Collaborative Care Transformation Organization, Inc. (MDCCTO) is an approved Care Transformation Organization(CTO) participating in the Maryland Primary Care Program (MDPCP). Since the first year (2019) of the MDPCP, the MDCCTO has partnered with MDPCP practices to achieve optimal health outcomes for their patients. We’re currently collaborating with 26 MDPCP practices on their care transformation journey.

Why MDCCTO?

MDCCTO has demonstrated success in advancing MDPCP practices’ care transformation. Our deep experience in practice management in managed care settings and interdisciplinary approach bolster our provider partners’ efforts to provide higher quality care and deliver optimal outcomes. MDCCTO empowers our partner practices to achieve better health for their patients by providing care management support across the continuum of care, including but not limited to:

• A robust medication management package with RxLive through which a pharmacist is fully integrated in the practice and provides comprehensive medication management with remote monitoring.
• A diverse interdisciplinary care coordination team of RNs, LPNs, social workers and community health workers for transition of care outreaches and case management.
• Community Connections Help Line, which helps connect patients to a wide range of services beyond healthcare to help meet any social determinants of health needs.
