Partner with Maryland Collaborative Care Transformation Organization (MDCCTO)

As an approved care transformation organization (CTO), MDCCTO has built the framework and established resources necessary to enable MDPCP practices of all sizes to deliver comprehensive primary care. In partnering with MDCCTO, practices will receive the support needed to meet MDPCP’s care transformation requirements and offer specialized care management to your patients.

The MDPCP CTO partnership selection period for PY 2025 is open now until Friday, September 6. Practices may choose a new CTO or change their current CTO partnership during this open enrollment period.

MDCCTO is available to partner with practices in all Maryland counties. Take the following steps to choose MDCCTO as your partner :

Interested in applying to the MDPCP?

There is no Request for Applications (RFA) for Performance Year 2025. The RFA issued in 2023 for Performance Year 2024 was the final RFA for MDPCP. Additional opportunities for new practices to join may exist under the upcoming AHEAD program.
